Things to bring to your office visit:
- 1. the completed New Patient Form printed from this website or mailed to you from our office
- 2. all your current medications
- 3. copy of your insurance card
- 4. current photo ID
- 5. name, phone number and fax number of your preferred pharmacy
- 6. name, phone number and fax number of your primary care physician and/or referring physician
- 7. your payment for your co-insurance and co-pay (due at time of visit).
